BACKGROUND: There is interest in lingering non-specific symptoms after severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection, referred to as Long coronavirus disease 2019 (Long COVID-19). It remains unknown whether the risk of Long COVID-19 is associated with pre-existing comorbidities or initial COVID-19 severity, including infections due to new Omicron lineages which predominated in 2023. OBJECTIVES: The aim of this case report was to characterize the clinical features of acute XBB.1.5 infection followed by Long COVID-19. METHODS: We followed a 73-year old female resident of Rio de Janeiro with laboratory-confirmed SARS-CoV-2 during acute infection and subsequent months. The SARS-CoV-2 lineage was determined by genome sequencing. FINDINGS: The participant denied comorbidities and had completed a two-dose vaccination schedule followed by two booster doses eight months prior to SARS-CoV-2 infection. Primary infection by viral lineage XBB.1.5. was clinically mild, but the participant subsequently reported persistent fatigue. MAIN CONCLUSIONS: This case demonstrates that Long COVID-19 may develop even after mild disease due to SARS-CoV-2 in fully vaccinated and boosted individuals without comorbidities. Continued monitoring of new SARS-CoV-2 lineages and associated clinical outcomes is warranted. Measures to prevent infection should continue to be implemented including development of new vaccines and antivirals effective against novel variants.

BACKGROUND: It is important to understand the dynamics of SARS-CoV-2 transmission in close-contact settings such as households. We hypothesized that children would most often acquire SARS-CoV-2 from a symptomatic adult caregiver. METHODS: This prospective cohort study was conducted from April 2020 to July 2022 in a low-resource, urban settlement in Brazil. We recruited families who brought their children to a public clinic. We collected nasopharyngeal and oral swabs from household members and tracked symptoms and vaccination. RESULTS: In total, 1256 participants in 298 households were tested for SARS-CoV-2. A total of 4073 RT-PCR tests were run with 893 SARS-CoV-2 positive results (21.9%). SARS-CoV-2 cases were defined as isolated cases (N = 158) or well-defined transmission events (N = 175). The risk of household transmission was lower if the index case was a child (OR: 0.3 [95% CI: 0.16-0.55], P < .001) or was vaccinated (OR: 0.29 [95% CI: 0.1-0.85], P = .024), and higher if the index was symptomatic (OR: 2.53 [95% CI: 1.51-4.26], P < .001). The secondary attack rate for child index cases to child contacts was 0.29, whereas the secondary attack rate for adult index cases to child contacts was 0.47 (P = .08). CONCLUSIONS: In this community, children were significantly less infectious to their household contacts than adolescents or adults. Most children were infected by a symptomatic adult, usually their mother. There was a double benefit of vaccination as it protected the vaccine from severe illness and prevented onward transmission to household contacts. Our findings may also be valid for similar populations throughout Latin America.

PURPOSE: To better understand the household transmission of SARS-COV-2 in a low-resource community in Rio de Janeiro during the COVID-19 pandemic (2020-2022). PARTICIPANTS: This is an open prospective cohort study of children ≤12 years old and their household contacts. During home visits over 24 months, we collected data on sociodemographic characteristics, behavioural data, clinical manifestations of SARS-CoV-2, vaccination status, SARS-CoV-2 (reverse transcription-polymerase chain reaction) RT-PCR and anti-S antibody tests. Among adults, the majority of participants were women (62%). FINDINGS TO DATE: We enrolled 845 families from May 2020 to May 2022. The median number of residents per household was four. The median household density, defined as the number of persons per room, was 0.95. The risk of SARS-CoV-2 occurrence was higher in households with a high number of persons per room. Children were not the principal source of SARS-CoV-2 infections in their households during the first wave of the pandemic. FUTURE PLANS: Future studies will investigate cellular and humoral immune responses to locally circulating SARS-CoV-2 variants, which is relevant for the design of vaccines, antivirals and monoclonal antibodies. We will also engage in outreach to encourage vaccination as a means of limiting the transmission of novel SARS-CoV-2 variants and other emerging pathogens.

This work aimed to prepare a nanoemulsion containing the essential oil of the Protium heptaphyllum resin and evaluate its biocidal activities against the different stages of development of the Aedes aegypti mosquito. Ovicide, pupicide, adulticide and repellency assays were performed. The main constituents were p-cymene (27.70%) and α-pinene (22.31%). The developed nanoemulsion showed kinetic stability and monomodal distribution at a hydrophilic-lipophilic balance of 14 with a droplet size of 115.56 ± 1.68 nn and a zeta potential of -29.63 ± 3.46 mV. The nanoemulsion showed insecticidal action with LC50 0.404 µg·mL-1 for the ovicidal effect. In the pupicidal test, at the concentration of 160 µg·mL-1, 100% mortality was reached after 24 h. For adulticidal activity, a diagnostic concentration of 200 µg·mL-1 (120 min) was determined. In the repellency test, a concentration of 200 µg·mL-1 during the 180 min of the test showed a protection index of 77.67%. In conclusion, the nanobiotechnological product derived from the essential oil of P. heptaphyllum resin can be considered as a promising colloid that can be used to control infectious disease vectors through a wide range of possible modes of applications, probably as this bioactive delivery system may allow the optimal effect of the P. heptaphyllum terpenes in aqueous media and may also induce satisfactory delivery to air interfaces.

The aim of this work was to prepare a nanoemulsion containing the essential oil of Protium heptaphyllum resin and to evaluate the larvicidal activity and the residual larvicidal effect against Aedes aegypti. The essential oil was identified by gas chromatography coupled to a mass spectrometer, and the nanoemulsions were prepared using a low-energy method and characterized by photon correlation spectroscopy. The results indicated the major constituents as p-cimene (27.70%) and α-Pinene (22.31%). Nanoemulsions had kinetic stability and a monomodal distribution in a hydrophilic-lipophilic balance of 14 with particle diameters of 115.56 ± 1.68 nn and zeta potential of -29.63 ± 3.46 mV. The nanoemulsion showed larvicidal action with LC50 = 2.91 µg∙mL-1 and residual larvicidal effect for 72 h after application to A. aegypti larvae. Consequently, the nanobiotechnological product derived from the essential oil of P. heptaphyllum resin could be used against infectious disease vectors.

RESUMO

In light of the 60-year anniversary of the publishing of "Clathrate Solutions" by van der Waals and Platteeuw in 2019, we present a critical review of the famed solid solution model first disclosed in 1959. First, we lay out the groundwork in the 1950s aimed at the development of a phenomenological approach to clathrate modeling. Then we review the statistical thermodynamics fundamentals of the model, considering van der Waals and Platteeuw's earlier works, to obtain a consistent interpretation of the model. We turn our focus to clathrate hydrates and discuss the major contributions that led to the current state-of-the-art of gas hydrate thermodynamic modeling. Finally, we present some of the areas in clathrate thermodynamics that we foresee as the new frontiers in this subject. We expect this review to help newcomers to clathrate science in elucidating some subtle aspects of the model and to intrigue clathrate experts with a fresh look on this well-established solid solution model.

La actinomicosis es una infección poco frecuente, lentamente progresiva y de difícil diagnóstico. Se presenta el caso clínico de una mujer de 63 años en remisión total de un linfoma no Hodgkin difuso de células B grandes de amígdala. Durante su seguimiento se observó un ligero aumento de la lactato deshidrogenasa (LDH). Por la posibilidad de una recaída de su linfoma se realizó una tomografía por emisión de positrones (TEP-TC) que detectó mayor densidad y actividad en la orofaringe izquierda. Se realizó la extirpación de la lesión, cuyo diagnóstico histopatológico fue de una actinomicosis amigdalina. El género Actinomyces es comensal de la orofaringe y de la vía respiratoria y digestiva. La forma cérvico-facial es la más frecuente. La extirpación quirúrgica es a menudo esencial para un diagnóstico definitivo. Es importante incluir esta enfermedad en el diagnóstico diferencial de las lesiones tumorales de la región cérvico-facial.

OBJETIVES: The main objective of the present randomized pilot study was to explore the effects of upstream prasugrel or ticagrelor or clopidogrel for patients with ST-segment-elevation myocardial infarction (STEMI) undergoing primary percutaneous coronary intervention (PCI). BACKGROUND: Administration of clopidogrel "as soon as possible" has been advocated for STEMI. Pretreatment with prasugrel and ticagrelor may improve reperfusion. Currently, the angiographic effects of upstream administration of these agents are poorly understood. METHODS: A total of 132 patients with STEMI within the first 12 hr of chest pain referred to primary angioplasty were randomized to upstream clopidogrel (600 mg), prasugrel (60 mg), or ticagrelor (180 mg) while still in the emergency room. All patients underwent protocol-mandated thrombus aspiration. RESULTS: Macroscopic thrombus material was retrieved in 79.5% of the clopidogrel group, 65.9% of the prasugrel group, and 54.3% of the ticagrelor group (P = 0.041). At baseline angiography, large thrombus burden was 97.7% vs. 87.8% vs. 80.4% in the clopidogrel, prasugrel, and ticagrelor groups, respectively (P = 0.036). Also, at baseline, 97.7% presented with an occluded target vessel in the clopidogrel group, 87.8% in the prasugrel group and 78.3% in the ticagrelor group (P = 0.019). At the end of the procedure, the percentages of patients with combined TIMI grade III flow and myocardial blush grade III were 52.3% for clopidogrel, 80.5% for prasugrel, and 67.4% for ticagrelor (P = 0.022). CONCLUSIONS: In patients with STEMI undergoing primary PCI within 12 hr, upstream clopidogrel, prasugrel or ticagrelor have varying angiographic findings, with a trend toward better results for the latter two agents. © 2015 Wiley Periodicals, Inc.

The phytochemical study of Cordia exaltata Lam. (Boraginaceae) led to the isolation, through chromatographic techniques, of nineteen secondary metabolites: 8,8'dimethyl-3,4,3',4'-dimethylenedioxy-7-oxo-2,7'cyclolignan (1), 8,8'-dimethyl-4,5-dimethoxy-3',4'-methylenodioxy-7-oxo-2,7'cyclolignan (2), sitosterol (3a), stigmasterol (3b), sitosterol-3-O-ß-D-glucopyranoside (4a), stigmasterol-3-O-ß-D-glucopyranoside (4b), phaeophytin A (5), 13²-hydroxyphaeophytin A (6), 17³-ethoxypheophorbide A (7), 13²-hydroxy-17³-ethoxypheophorbide A (8), m-methoxy-p-hydroxybenzaldehyde (9), (E)-7-(3,4-dihydroxyphenyl)-7-propenoic acid (10), 1-benzopyran-2-one (11), 7-hydroxy-1-benzopyran-2-one (12), 2,5-bis-(3',4'-methylenedioxiphenyl)-3,4-dimethyltetrahydrofuran (13), 3,4,5,3',5'-pentamethoxy-1'-allyl-8.O.4'-neolignan (14), 3,5,7,3',4'-pentahydroxyflavonol (15), 5,7-dihydroxy-4'-methoxyflavone (16), 5,8-dihydroxy-7,4'-dimethoxyflavone (17), kaempherol 3-O-ß-D-glucosyl-6''-α-L-ramnopyranoside (18) and kaempherol 3,7-di-O-α-L-ramnopyranoside (19). Trata-se de um relato de caso de tumor de células gigantes (TCG) em falange proximal de terceiro dedo da mão esquerda com metástase pulmonar. A paciente apresentava dor no dedo sem história prévia de trauma. Foram realizados exame clínico, estudo radiográfico e ressonância nuclear magnética. Feito o estudo histológico, a partir de biópsia incisional, com hipótese de TCG. Foi submetida à amputação do dedo, confirmando o diagnóstico pela microscopia da peça. A paciente foi acompanhada devido ao risco de metástase pulmonar, evidenciada em estudo radiográfico e tomografia computadorizada de tórax, sendo submetida à toracotomia. Desde então, houve melhora dos sintomas referidos no pré-operatório e ausência de recidiva local e novas metástases.

This is a case report on a giant cell tumor (GCT) in the proximal phalanx of the third finger of the left hand, with pulmonary metastasis. The patient presented pain in the finger without any previous history of trauma. Clinical examination, radiographic imaging and magnetic resonance imaging were carried out. A histological evaluation was carried out from an incisional biopsy, taking the hypothesis of GCT. The patient underwent amputation of the finger and the diagnosis was confirmed by means of microscopy on the specimen. The patient was followed up because of the risk of lung metastasis, which was shown by radiographic examination and computed tomography on the chest, and thoracotomy was performed. Since then, there has been an improvement in the symptoms that had been reported preoperatively, and no local recurrence or new metastasis has been found.

Pradosia huberi (Ducke) Ducke (Sapotaceae), an Amazonian species, is popularly known as "casca-doce" and used in the folk medicine for the treatment of gastritis. The ethanol extract of the bark contains mainly polyphenolic compounds, which are known to show a large number of activities, including cardioprotective and vasorelaxant effects. The aim of this study was to evaluate the pharmacological properties induced by P. huberi ethanol extract (PHEE) and fractions and 2-3-dihydromyricetin-3-O-α-L-rhamnoside derived from this extract, in isolated rat mesenteric arteries. PHEE was separated and the following fractions were obtained: CHCl3, CHCl3:AcOEt (1:1), AcOEt, AcOEt:MeOH (1:1) and MeOH. We isolated 2-3-dihydromyricetin-3-O-α-L-rhamnoside from the MeOH fraction, which was identified by¹H and 13C NMR spectra and compared with data in the literature. PHEE (1-100 µg/mL) induced concentration-dependent relaxations of 10 µM phenylephrine-induced tone (EC50=17,1±2,9 µg/mL; Emax=87.4±2.9 percent, n=8). The MeOH fraction also relaxed mesenteric rings (EC50=31±2.0 µg/mL; Emax=54±12.5 percent, n=6) but less effectively when compared to PHEE. Both effects were completely abolished after removal of the vascular endothelium. The AcOEt:MeOH (1:1) fraction and the isolated flavonoid were ineffective in eliciting vasorelaxation. The study demonstrates that PHEE and MeOH fraction of Pradosia huberi possess a vasorelaxant effect, which may be completely dependent upon endothelium. The isolated flavonoid is not responsible for this vasorelaxant effect.


Pradosia huberi (Ducke) Ducke (Sapotaceae), espécie Amazônica popularmente conhecida como "casca-doce" é utilizada na medicina tradicional no tratamento de gastrite. O extrato etanólico de suas cascas é rico em polifenóis que podem apresentar um grande número de atividades, incluindo efeito vasorelaxante e cardioprotetor. O objetivo deste estudo foi avaliar as propriedades farmacológicas do extrato etanólico (EPH), de frações e da 2,3-diidromiricetina-3-O-α-L-raminosídeo isolados de P. huberi, em artéria mesentérica isolada de rato. O EPH foi fracionado resultando nas seguintes frações: CHCl3, CHCl3:AcOEt (1:1), AcOEt, AcOEt:MeOH (1:1) e MeOH. Da fração MeOH foi isolada a 2,3-diidromiricetina-3-O-α-L-raminosídeo e identificada através de espectro de RMN de ¹H e 13C, além de comparações com os dados de literatura. EPH (1-100 µg/mL) promoveu relaxamento dependente de concentração no tônus vascular induzido por 10 µM de fenilefrina (CE50=17,1±2,9 µg/mL; Emax=87,4±2,9 por cento, n=8). A fração MeOH também relaxou os anéis mesentéricos (CE50=31±2,0µg/mL; Emax=54±12,5 por cento, n=6), porém com menor eficácia quando comparado ao efeito de EPH. Tanto o efeito de EPH com de MeOH foram completamente abolidos após a remoção do endotélio vascular. A fração AcOEt:MeOH (1:1) e o flavonoide isolado induziram vasorelaxamento. O estudo demonstrou que o EPH e a fração MeOH de Pradosia huberi apresentam propriedade vasorelaxante que pode ser completamente dependente da presença do endotélio. O flavonoide isolado não é o responsável por este efeito vasorelaxante.

OBJECTIVE: To clinically and anatomically compare the limits of the fibular nerve in Gerdy's safe zone in cadavers. METHODS: Fifty anatomical knee specimens were clinically measured (before dissection) to determine the distances and angles between Gerdy's tubercle and the posterior region of the fibula (cm); the angle between the line corresponding to the posterior region of the fibula and the tibial crest (degrees); and the angle between Gerdy's tubercle and the tibial crest (degrees). After dissection of the anatomical specimens, the knees were measured again, to determine the distances and angles between Gerdy's tubercle and the fibular nerve (cm); the angle between the fibular nerve, in the posterior region of the fibula, and the tibial crest (degrees); and the angle between Gerdy's tubercle and the tibial crest (degrees). RESULTS: There was a significant increase in the distance between Gerdy's tubercle and the posterior region of fibula (cm), after dissection: an average of 0.26 cm (p = 0.018), corresponding to 8.6% (p = 0.007). There was also a significant reduction in the angle between the posterior region of the fibula and the tibial crest after dissection: an average of 3° (p = 0.047), corresponding to 2.1% (p = 0.06). CONCLUSION: Despite the difference from before to after dissection, Gerdy's safe zone can be considered safe for orthopedic procedures involving the proximal region of the tibia, thereby avoiding damage to the fibular nerve and its branches.

OBJETIVO: Comparar clínica e anatomicamente os limites do nervo fibular na zona de segurança de Gerdy em cadáveres. MÉTODOS: Foram utilizados 50 espécimes anatômicos de joelhos e medidos clinicamente (antes de dissecção) para determinar as distâncias e ângulos entre: o tubérculo de Gerdy e a região posterior da fíbula (cm), ângulo entre a linha correspondente à região posterior da fíbula e a crista tibial (graus) e o ângulo entre o tubérculo de Gerdy e crista tibial (graus). Após a dissecção das peças anatômicas, os joelhos foram novamente medidos, para determinar as distâncias e ângulos entre: o tubérculo de Gerdy e o nervo fibular (cm), ângulo entre o nervo fibular, na região posterior da fíbula e a crista tibial (graus) e o ângulo entre o tubérculo de Gerdy e crista tibial (graus). RESULTADOS: Existe aumento significativo da distância entre o tubérculo de Gerdy e a região posterior da fíbula (cm), após dissecção, em média 0,26cm (p = 0,018), o que corresponde a 8,6 por cento (p = 0,007) e também existe diminuição significativa do ângulo entre a região posterior da fíbula e a crista tibial após dissecção, em média de 3º (p = 0,047), que corresponde a 2,1 por cento (p = 0,06). CONCLUSÃO: Apesar da diferença antes e depois da dissecção, a zona de segurança de Gerdy pode ser considerada segura para os procedimentos ortopédicos, na região proximal da tíbia, evitando danos ao nervo fibular e seus ramos.

RESULTS: There was a significant increase in the distance between Gerdy's tubercle and the posterior region of fibula (cm), after dissection, of an average 0.26 cm (p = 0.018), corresponding to 8.6 percent (p = 0.007). There was also a significant reduction in the angle between the posterior region of fibula and the tibial crest after dissection, of an average 3º (p = 0.047), corresponding to 2.1 percent (p = 0.06). CONCLUSION: Despite the difference before and after dissection, Gerdy's Safety Zone can be considered safe for orthopedic procedures involving the proximal region of the tibia, preventing damage to the fibular nerve and its branches.

RESUMO

The cardiovascular effects induced by the hydroalcoholic extract of the stem of Xylopia cayennensis (HEXC) were studied in rats using a combined in vivo and in vitro approach. In non-anesthetized rats, HEXC injections produced a significant and dose-dependent hypotension associated with an increase in heart rate. The hypotensive response was not attenuated after nitric oxide (NO) synthase blockade, L-NAME (20 mg/Kg, i.v.). In isolated rat superior aortic rings, HEXC was able to relax the tonus induced by phenylephrine (1 µM) and KCl (80 mM), (EC50 = 85±13 and 62±5 µg/mL, respectively). The smooth muscle-relaxant activity of HEXC was not inhibited by removal of vascular endothelium (EC50 = 58±6 µg/mL). HEXC antagonized CaCl2-induced contractions in depolarizing medium nominally without Ca2+. HEXC inhibited the intracellular calcium-dependent transient contractions induced by caffeine (20 mM) in Ca2+-free solution, but not those induced by norepinephrine (1 µM). In isolated rat atrial preparations, HEXC produced negative inotropic and chronotropic responses (IC50= 534±42 and 259±22 µg/mL, respectively). The results obtained suggest that the hypotensive effect of HEXC is probably due to a peripheral vasodilatation, at least, secondary to an interference with the Ca2+ mobilization as a consequence of voltage-dependent Ca2+ channel blockade and the inhibition of Ca2+ release from caffeine-sensitive intracellular stores. Finally, HEXC acts directly on the heart decreasing contractility and heart rate, these effects are of little importance to the expression of the hypotensive response induced by HEXC.


Os efeitos cardiovasculares induzidos pelo extrato hidroalcoólico do caule de Xylopia cayennensis (EHXC) foram estudados em ratos, utilizando uma abordagem combinada in vivo e in vitro. Em ratos não anestesiados, EHXC induziu uma hipotensão não dependente de dose associada com um aumento da freqüência cardíaca. Esta resposta hipontesora não foi atenuada depois do bloqueio com L-NAME (20 mg/Kg, i.v.). Em anéis de aorta isolados de rato, EHXC foi capaz de relaxar o tônus induzido por fenilefrina (1 µM) e KCl (80 mM), (CE50 = 85±13 e 62±5 µg/mL, respectivamente). A atividade vasorelaxante do HEXC não foi inibida pela remoção do endotélio vascular (CE50 = 58±6 µg/mL). HEXC antagonizou as contrações induzidas por CaCl2 em meio despolarizante nominalmente sem Ca+2. EHXC antagonizou de maneira dependente de concentração as contrações transientes induzidas por cafeína (20 mM), em meio livre de Ca2+, contudo não alterou aquelas induzidas por noradrenalina (1 µM). Em átrio isolado de rato, EHXC induziu um efeito inotrópico e cronotrópico negativo (CI50= 534±42 µg/mL e 259±22 µg/mL; respectivamente). Os resultados obtidos demonstram que EHXC apresenta um potente efeito hipotensor, provavelmente conseqüência da diminuição da resistência periférica total que parece ser, em parte, devido a uma ação inibitória sobre o influxo de Ca+2 através de canais de cálcio dependentes de voltagem e também através da inibição da liberação de Ca+2 dos estoques intracelulares sensíveis à cafeína. HEXC atua diretamente no coração diminuindo a contratilidade e a freqüência cardíaca, estes efeitos têm importância pequena na expressão da resposta hipotensiva induzida por HEXC.
